Creating a New Questionnaire Template
As previously mentioned, You can import a template via uploading an Excel file. If no such file exists, create a new template or import one from Excel.
Start by clicking on the Questionnaire Details button , on the top right corner.
In the new screen you'll be able to give the Questionnaire a meaningful name, set up it's language and give detailed, explicit instructions that will be attached to the questionnaire.
Then, let's start adding questions.
Begin by selecting one of the available question types from the list below:
- Yes/No question
- Open-ended question (Notice - Open-ended questions do not support Logic)
- Multiple Choice question
- Document Request (This asks the supplier to upload a specific document)
- Question Group
You can customise each section by adding a label and an optional description, as well as configuring its settings.
Customization Options for Questions
A variety of options are available for customizing each question.
- Comment - This enables a comments section to be added beneath the question, allowing the supplier to provide additional input.
- Mandatory - This indicates that this section MUST be answered, as indicated by a red star.
- Document attachment - This allows suppliers to attach relevant files to their response.
- Character limit - This feature allows you to specify the maximum number of characters to be used for open-text responses, ensuring conciseness.
-
Choice Restriction - (For multiple-choice questions) allows you to specify how many answers can be selected (e.g. one choice only or a range such as 1–2).
Adding Logic to Questionnaires
Use the Logic Settings feature to design dynamic questionnaires.
This will allow you to hide specific questions based on suppliers' answers, which reduces overall form complexity.
